WHAT DOES THE SOCIAL WORKER DO?

Compassionate Support for Residents and Staff in Long-Term Care

Social Workers play a vital role in supporting both residents and staff in ong-term care communites. With a strong background in gerontology and experience in geriatric care, they bring the empathy and expertise needed to enhance the well-being of our residents. As a dedicated team member in a multidisciplinary setting, they navigate various responsibilities assigned by the facility, ensuring comprehensive support and compassionate care.

ABOUT THE SERVICE:

The Social Worker will incorporate all the fundamentals needed to aid residents and staff in a long term care setting.

  • They will have a background in Gerontology
  • They will have experience and the empathy required to work in a geriatric setting
  • They will work as a“team member” in a multidisciplinary setting.
  • They will navigate through the different areas and responsibilities as assigned by the facility. 

We arrange social workers that are trained to address mental, emotional and physical needs of residents. They may work in group settings or in individual appointments, depending on the residents needs

HOW IS HAVING A SOCIAL WORK TEAM BENEFICIAL?

  • Collaborate with the nursing team, physicians, residents, and families to provide comprehensive care.
  • Serve as a liaison between CCAC and the long-term care facility. Provide individual therapy and group counselling, with a focus on Dementia and Alzheimer’s residents.
  • Lead meetings conferences, and workshops, including Resident and Family Council Meetings.
  • Conduct client assessments to determine service needs and ensure appropriate levels of care.

FACT:

Our Goal is to utilize the skills and experience of the Social Worker to work as a “team member” in a multidisciplinary setting.  The Social Worker will navigate through the different areas and responsibilities as assigned by the facility.
